Based on observation at 9:41 a.m., children failed to be under supervision at all times. The Specialist observed C1, 3-year-old, unsupervised in the center's office. C1 remained unsupervised until S1 returned to the office at 9:42 a.m. Corrective Action: Effective 5/5/2026, S1 will not allow children to be left in the office with her to ensure compliance with this regulation.

Based on observations at 10 a.m., S1 failed to keep items that can be harmful to children, such as cleaning supplies and chemicals, in a locked cabinet or other secure place that ensures they are inaccessible to children. The Specialist observed 3 cans of disinfectant spray, 2 spray bottles of cleaner, and a container of disinfecting wipes in an unlocked cabinet and accessable to 7 children, age 1-year-old, in S11's classroom. Based on record review, at 9:45 a.m., S1 failed to have documentation that all staff have current certification in CPR and Pediatric First Aid through approved by the Department within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. 1919.D.2. Based on observation at 9:50 a.m., S10 failed to ensure children under age 4 were not served food implicated in choking incidents. The Specialist observed 7 children, ages 2-years-old, eating hard pretzels in S10's classroom. This was corrected prior to the Specialist exiting the center. 1103.A.5.&11.&B.1.&C.1.&2.: Based on record review and interviews on 3/16/2026, at 12 p.m., S1 failed to notify the parent immediately and Department and DCFS within 24 hours of the following critical incidents: ¨ On 3/11/2026, at 10 a.m., during recess, C1, 3-year-old, escaped through an unsecured gate on the play yard, resulting in him being outside the licensed space. 1509.A.8.a.&b.i.: Based on record review and interviews on 3/16/2026, at 1 p.m., although the center has a behavior management policy in place that prohibits placing a child in an uncomfortable position, S10 (DOH: 10/29/2025) used a prohibited method of discipline where on 3/11/2026 or 3/12/2026, S9 observed S10 place her legs on top of C1's, 3-year-old, back during nap to keep him from moving and getting up. C1 was crying/screaming and appeared to be in distress. S10 stated this incident never took place, but was written up on 3/11/2026, for having her "legs over a child during nap time."…

Based on interviews on 3/16/2026, at 1 p.m., the outdoor play space lacked an enclosure with a permanent fence or barrier in a manner that protected children from traffic hazards, prevented children from leaving the premises without proper supervision, and prevented contact with animals or unauthorized persons. On 3/11/2026, at 10 a.m., C1, 3-year-old, escaped through an unsecured fence gate, resulting in him leaving the play yard and accessing unlicensed space, located next to a street. Based on observation at 11:15 a.m., the center's cribs failed to be free of toys or other soft or loose bedding (including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges) while the child was in the crib. The Specialist observed two children in cribs with loose blankets and one child in a crib with a loose blanket and a pillow. This was corrected during the inspection.

Based on observations at 11:15 a.m., infants failed to be placed on their backs for sleeping as the specialist observed S4 place C1, five-month-old, on his stomach in the crib. This wascorrected during the inspection.
