1713.A.&B.&C.: Based on observation at approximately 2:00 p.m., S2 failed to provide supervision to children at all times. The Specialist rang the doorbell of the center to gain entry. S2 came to the door, saw the Specialist through the window, and then left the door without opening it. Upon gaining entry to the center the Specialist identified that S2 was responsible for four children (C1, C2, C3, and C4) ages, three to eleven-months-old. Based on observation at approximately 2:20 p.m., written authorization from a physician was not available for 1 of 4 infants as required for infant to sleep in a car seat or other similar device. Specialist observed that C1, 3-months-old, was asleep in a baby swing and not in her assigned crib. There was no written authorization for sleeping for C1. Based on observation at approximately 2:15 p.m., S2 allowed an infant's (C1, 3-months-old) head/face to be covered by a navy blue blanket. S2 removed the blanket from C1's face prior the Specialist leaving the center. Corrective Action: Effective 3/20/2026, S1 stated a memo will go out to parents that blankets are no longer allowed in the infant room as of 3/31/2026, to ensure compliance with this regulation.

Based on record review at approximately 2:15 p.m., S1 failed to ensure the transportation arrangements conform to state laws, as the center vehicle registration was expired upon inspection. The registration expired on 2/28/2026. S7 transported children on the following days with expired registration: ¨ The weeks of 3/2/2026-3/6/20026, 3/10/2026-3/13/2026, 3/16/2026-3/20/2026, and 3/23/2026-3/27/2026.
