      1103-A.5.C.1.2. Based on record review/interview at 1:15 p.m., S1 failed to notify the Department and DCFS/Child Welfare within 24 hours of the following critical incident: On 9/19/2025, at 5:24 p.m., S1 was notified by O1 that C1, a one-year-old, was picked up at 5:02 and had a foul smell. S8 informed O1 that it was dirt. When O1 took off C1's diaper, dry fecal matter was observed and caused a rash. Corrective Action: Effective 10/02/2025, S1 stated she will submit all critical incidents to the department and DCFS upon notification to ensure compliance to this regulation.
