CCF Handbook, Section 3.1 (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] All ceiling air vents and areas around air vents in facility were observed to be caked with dust that could impact childrens breathing. Some vents and ceiling areas are worse than others. Director had requested a corporate work order for this on 4/8/26, 3.1 General Health and Safety Requirements page 20 A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. Staff member has signed up and will be retaking test 8.16.25. 4.1 Beginning Training page 39-40 Childcare personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the childcare industry and successfully complete Departments training within 12 months from the date training begins. The facility did not have electrical outlet covers or the outlet covers were not in place. CCF Handbook, Section 3.1, I (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] 4 classrooms in use at this time, did not have covers on electrical outlets. The outlets were installed higher up that smaller children could not reach however older children could possibly reach. Director is aware and has plug covers on order. Corrections will be made by 5/16/25 and Director will contact Specialist when completed. 3 Physical Environment page 20-21 Children are much more susceptible to the adverse effects of environmental factors, materials and toxicants. It is imperative that the child care environment is conducive to the healthy development of children. I. All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. In settings that do not have tamper-resistant electrical outlets, outlets shall have safety covers.

(Section 3, numbers 1 9) General Requirements [SR] At 8:53 am counselor walked into inside gym and found seven 1 to 2 years olds with 1 staff member. Director was giving a tour of facility to a perspective family. Ratio was in compliance when a director moved a 2-year-old to 3-year-old classroom. This was corrected at 8:59am. 2.3 Ratios page 12 The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. Child was unharmed and parents notified immediately. 2.4 Supervision page 12 2.4.1 General Supervision Requirements B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group.
