Based on interviews on 4/16/2026, at 12:40 p.m., S5 failed to ensure while supervising a group of children that she devoted her time to supervision of the children, meeting the needs of the children, and in participation with the children in their activities. S5 stated she was cleaning and organizing the cabinet with her back turned towards the children in her classroom from 4:15 p.m. to 5:00 p.m. Although S5 would address the children to correct their behavior, she would then turn back around to continue her cleaning. Based on record review and interviews at 1:30 p.m., S1 failed to ensure staff did not use a prohibited method of discipline as C1, aged 3-years-old, was subject to physical punishment and verbal abuse. On 12/19/2024, at 3:45 p.m., S13 (DOH 10/23/2023) pulled C1 by the arm and yelled at her for not lining up to go inside from the playground.
