      Based on record review and interviews at 2:45 p.m., S1 failed to notify LDOE and Child Welfare within 24 hours of the following critical incident : On 1/22/2026, at 2:44 p.m., O1 informed S2 that C2, aged 4-years-old, informed her that C1, aged 2-years-old, was being pinched and slammed into her chair by S2. S2 informed S1 about the allegation immediately after. S1 did not notify the Department and DCFS about the allegation. Specialist was unable to validate the allegation.       1103.A.9 Based on interviews at 11:11 a.m., S1 failed to notify within 24 hours of the incident the Department and other appropriate agencies of the following critical incident a physical altercation between adults in the presence of children on the premises. S1 stated that a verbal altercation between S9 (DOH 04/25/2025) and S11 (02/07/2025) occurred on May 12, 2025 in front of the children. Corrective Action: Effective 05/28/2025, S1 stated she will notify LDOE and DCFS Child Welfare with all Critical Incidents that occur to ensure compliance with the regulation.

During the walk-through, the Specialist observed C2, age two-years-old, alone in the centers hallway unsupervised. S2 retrieved C1 at 9:28 a.m., and walked her back to her classroom with his teachers S7. Corrective Action: Effective 05/28/2025, S1 stated she will have a meeting with staff to discuss active supervision to ensure no child is left unsupervised to ensure compliance with this regulation.

      1911.G. Based on observations on 05/28/2025, at 9:33 a.m., S5 failed to ensure pacifiers were not attached to the clothing of an infant. C3, age 7 months, was observed with a pacifier clipped to his clothing. S4 removed the clip during the licensing visit. Corrective Action: Effective 05/28/2025, S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.
