      Based on record review and interviews on 05/14/2026, at 12:06 p.m., S21 failed to ensure children were under supervision at all times. On 05/11/2026 at 9:26 a.m., C1, age 2-years-old, was left alone and unsupervised for approximately 30 minutes on the Young Two's play yard as S21 transitioned her class inside to the gym. C1 was retrieved by S4, at 10:02 a.m. S21 was terminated. Corrective Action: Effective 05/14/2026, S1 stated she will ensure that all children are supervised at all times to ensure compliance with this regulation.

      1103.B.3 Based on interviews and record review at 11:00 a.m., S4 and S7 failed to notify the parent immediately of the following critical incident: On 11/21/2025 at approximately 9:50 a.m., S4 and S7 observed S3 repeatedly pulling down C1’s pants each time he pulled them up during play time. O1 was notified on 11/24/2025 at 9:20 a.m. Corrective Action: Effective 12/9/2025 S1 stated she retrain all of her staff on mandated reporting to ensure compliance with this regulation.

      1713.A. Based on observations at 9:18 a.m., S8 failed to ensure children were under supervision at all times. The Specialist observed S8 with her back turned, standing next to S9 while S9 attended to an infant on the changing table in the changing room, which resulted in the remaining 4 children, ages 2 months-to-1-year-old, being left unattended on the floor on the other side of the changing room’s half wall.
