Based on record review/interview at 3:30 p.m., S1 failed to ensure that S20, S23 and S28 had current certification in pediatric first aid and CPR within 90 days from the date of hire and prior to assuming sole responsibility for any children. The Specialist observed S20 (DOH 6/2/2025), solely responsible for 6 children, all 1-years-old and S23 (DOH 6/30/2025), solely responsible for 5 children, ages 12-18 months. S28 (DOH 3/5/2025) failed to have required pediatric first aid and CPR training within 90 days from date of hire. This was not corrected prior to the Specialist departure.…

Based on record review/interviews at 2:00 p.m., S30 failed to ensure children were supervised at all times. On 3/25/2025, C1, 2.3-years-old, was left unsupervised for 28 seconds, from 4:28:24 p.m.-4:28:48 p.m. O1 arrived on the hallway and picked up C1. This could not be corrected prior to the Specialist departure. Corrective Action: Effective 4/29/2025, S1 stated she will reinforce the face-to-name policy every 15 minutes and when children are walking, a teacher must hold their hands to ensure compliance with this regulation.
