      Based on observations at 10:22 a.m., S1 and S2 failed to ensure the outdoor areas eres free of hazards. The Specialist observed the following on the center's playground: the grass was overgrown, a deflated inflatable toy which presented a tripping hazard and a disassembled swing set. This was not corrected prior to the Specialist departure. Corrective Action: Effective 6/2/2026, S1 stated due to the weather, they have not been able to properly clean the playground and the children have not played outside.       Based on record review at 12:30 p.m., S1 and S9 failed to ensure the passenger transportation for 5/4/2026-5/8/2026, included the pick up and drop off location of children on the center's vehicle. Also, on 5/8/2026, S9 failed to include the pick up and drop off locations, time child was placed on the vehicle and the time child was released and name of the person or entity to whom child was released for C1 and C3-C4, all 5-years-old, and C2, 6-years-old.       Based on observations at 1:00 p.m., S10 failed to ensure children were supervised at all times. The Specialist observed 17 children, ages 5 to 15-years-old alone in the classroom at rest time when S10 stepped out of the classroom. This was corrected at 1:02 p.m., when S10 re-entered the classroom. Corrective Action: Effective 3/12/2026, S2 stated she will make sure all classes are supervised before a teacher steps out to ensure compliance with this regulation.

      C.: Based on record review and interview, S2 failed to have documentation that S3 and S8 had a minimum of 12 clock hours per center anniversary year. S3 had 10.75 hours and S8 had 7.75 hours, This was not corrected prior to the Specialist departure. Corrective Action: Effective 3/12/2025, S2 stated she will schedule continuing education classes prior to the last month of the anniversary year to ensure compliance with this regulation.
