Based on interviews and record review, S1 failed to immediately notify the parent when the following incident occurred to a child: On 06/02/2026 at approximately 3:40 p.m., C1, 4-years-old, was crawling around on the classroom floor and bumped his head on a classroom table, resulting in a knot on the back of his head. Based on observations on 05/18/2026, at 12:45 p.m., S1 failed to ensure the playground was free of crawlspaces. The Specialists observed missing, loose, and damaged skirting along bottom of the 3 and 4-Year-Old Classroom, facing the playground. This could not be corrected prior to the Specialists' departure. Corrective Action: Effective 05/28/2026: S1 stated she will have the loose skirting repaired, and she and the lead teachers will be responsible for inspecting the playground for crawlspaces to ensure compliance with this regulation.

Based on record review/interview at 11:45 a.m., S4 (DOH: 3/3/2025) used a prohibited method of discipline as C1, 1-years-old was subjected to physical punishment. On 9/17/2025, at 2:32:10 p.m. S4 pulled C1 by the right arm while on the cot, causing him to hit head on the edge of the cot resulting in a cut over his left eye. On 9/17/2025, at 2:32:10 p.m., S4 did not immediately respond to C1, 1-years-old, as he laid crying for approximately 10 minutes on his cot at rest time. On 9/17/2025, at 2:32:10 p.m., S4 lifted C1, 1-years-old, by one arm onto his cot causing a cut over his left eye. S4 was immediately terminated. On 4/17/2025, C1, 5-years-old, and C2, 3-years-old, were signed in the centers after school age room at 3:16 p.m., but they did not enter the center because they were left on the center's van by S3. Based on record review on 4/25/2025, at 12:00 p.m., S1 failed to ensure that S9 had current certification in pediatric first aid and CPR within 90 days from the date of hire and prior to assuming sole responsibility for any children. The Specialist observed S9 (DOH: 3/3/2025) solely responsible for 4 children, ages 1-years-old. This was not corrected prior to the Specialist departure.
