Based on record review on 01/14/2026, at 2:00 p.m., S3 failed to have a signed and dated parental authorization to secure emergency medical treatment for C1, 4-years-old. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 01/14/2026, S3 stated she will ensure the parent signs emergency medical treatment authorization, and she will make a reminder for herself to ensure all new enrollee applications are fully filled out in order to ensure compliance with this regulation.

Based on record review on 01/14/2026, at 2:00 p.m., S3 failed to have documentation on file for O1 and O2 that included their name, address, phone number, and list of duties performed while at the center. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 01/14/2026, S3 stated she will complete the missing forms from O1 and O2's file, and she make a list of all current independent contractors so when the center gets a new independent contractor, she will know to create the new file to ensure compliance with this regulation.

Based on observation/record review on 12/08/2025, at 9:00 a.m., S2 failed to ensure the center's daily attendance record for children accurately reflected the children on the child care premises at any given time. There were 6 children present and 7 children were signed in on the log. This was corrected prior to the Specialist's departure. Corrective Action: Unable to provide a corrective action plan.

Based on observation/record review on 12/01/2025, at 9:00 a.m., S1 failed to ensure the center's staff and owner's daily attendance record accurately reflected persons on the child care premises at any given time. S1 was signed in on the log for 7:30 a.m., but was not on the premises. This was corrected when S1 arrived at the center at 10:41 a.m. On 12/08/2025, S1 and S4 were signed in on the log for 7:30 a.m., but were not on the premises. The Specialist observed S3 supervising 9 children, aged 7-months-old to 5-years-old, alone. S3 could only supervise 5 children. There needed to be an additional staff in the class with S3. The Specialist observed S2 leave C1, 7-months-old, in a classroom asleep in a crib, alone. She stated S3 was going to check on C1 and supervise her class. S3 was initially responsible for 6 children, ages 3 to 4-years-old. Around 10:15 a.m., S2 was cooking lunch and a parent arrived at the center. C1 was in the bouncer upon the Specialist arrival at the center at 9:00 a.m. and remained in the bouncer until 11:00 a.m. This was corrected prior to the Specialist's departure. Results including an explanation from parent and/or child were not documented. S2 stated they normally complete the observations at the end of the day.
