Based on record review, observation and interviews on 04/27/2026 at 11:00 a.m., S16 and S26 failed to ensure children were supervised at all times. On 04/20/2026, at 4:40 p.m., C1 and C2, both 2-years-old, opened the classroom door and walked out of the classroom. C1 and C2 remained in the hallway and bathroom area unsupervised for approximately 8 minutes until S16 and S26 realized they were not in the classroom. 1723.A.&B.: Based on record review at 12:15 p.m., S1 failed to have documentation 1 of 37 staff on the premises and accessible to children have current certification in CPR through training approved by the Department. S24 failed to have the current certification. A class has been scheduled for 12/29/2025. Corrective Action: Effective 12/18/2025, S1 stated S24 was on the list for the training but was overlooked to human error. They will be more aware of due dates to ensure compliance with this regulation.

On 09/16/2025, C1, 1- year-old, was left unsupervised in the lobby for approximately 40 seconds between 2:52 p.m. and 2:53 p.m. when the class exited the building to cross the street on their way to the gym. S15 entered the lobby and noticed C1 was left alone, then brought C1 to join his class in the gym. C1 was not upset and was unharmed. Based on record review at 12:00 p.m., S1 failed to have documentation of current CPR/Pediatric First Aid within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children for S16 (DOH 11/4/2024). This was not corrected prior to Specialist departure.

Based on interview at 1:00 p.m., the director failed to notify the Department within 24 hours after a complaint was received on 9/18/2024 that S24 was observed on camera grabbing a 6-months-old, by his chin and jerking his head back and forth because he refused to feed. S24 was also observed grabbing C1 roughly off the floor. Per the director, she stated she reviewed the camera footage and determined S24 was frustrated and didn't hurt or harm the child.

1507.E Based on record review/interviews at 1:00 p.m., S1 failed to ensure the daily attendance records for visitors accurately reflected the visitors on the center premises at any given time. S1 stated O2 visited the center on 12/9/2024 to investigate a critical incident. She was not signed in or out on the visitors log. This was not corrected prior to the Specialist departure.
