Based on record review at 11:00 a.m., S1 failed to have documentation 1 of 25 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S22 failed to have the current certification. S22's CPR expired 4/30/2026. A class has been scheduled for 6/15/2026. Corrective Action: Effective 6/8/2026, S1 stated she will keep up CPR and PFA will keep up expiration dates and make sure all staff has the training to ensure compliance with this regulation.

Based on record review at 11:00 a.m., S1 failed to ensure S2 had current certification in Pediatric First Aid and CPR within 90 days from the date of hire. S2's date of hire was 2/9/2026. A class has been scheduled f06 6/15/2026. Corrective Action: Effective 6/8/2026, S1 stated she will keep up CPR and PFA will keep up expiration dates and make sure all staff has the training to ensure compliance with this regulation.

Based on observation at 10:04 a.m., S1 failed to ensure the transportation arrangements conformed to state laws. Van 1 failed to have a current registration sticker. The last registration expired 5/2026. This was not corrected prior to the Specialists departure. Corrective Action: Effective 6/8/2026, S1 stated she will contact OMV to inquire about getting registration sticker and will check regularly for expiration date to ensure compliance with this regulation.

Based on observations at 10:30 a.m., S1 failed to ensure the indoor areas were free of hazards as the Specialist observed the electrical outlet in the Preschool 1 Classroom was missing an outlet cover. There were exposed wires and a broken socket cover with an exposed sharp edge. This was not corrected prior to the Specialist departure. Corrective Action: Effective 4/8/2026, S1 stated she is working with the church to have an electrician fix the socket ASAP to ensure compliance with this inspection.

1901.J.&K.: Based on observation at approximately 10:00 a.m., S10 and S11 failed to ensure items that can be harmful to children, such as cleaning supplies and plastic bags were kept in a locked cabinet or other secured place that ensured they were inaccessible to children. In the 2-year-old classroom, the Specialist observed 1 can of Febreeze spray and 1 roll of black trash bags under the classroom sink in an unlocked cabinet in reaching distance of the children. Based on observation at approximately 10:00 a.m., the indoor and outdoor areas failed to be free of hazards as there were 5 light blue chairs on top of the table in S10 and S11's classroom. There was 1 electrical outlet that was not covered and did not have a plastic cover to secure it to the wall in S15’s classroom. On the toddler playground, the brown, heavy duty, plastic fence contained 3 cracked/open areas exposing rough plastic. The stacked chairs were corrected prior the Specialist leaving the classroom. Based on observation at 11:15 a.m., S15 and S16 failed to ensure items that can be harmful to children was kept in a locked cabinet or other secure placed that ensures they are inaccessible to children. The Specialist observed the cabinet under the sink unlocked and contained 2 empty plastic bags, 1 bottle of Windex, 2 spray bottles of disinfectant spray, 1 tub of Clorox disinfectant cleaner, Swiffer wet cloths, BETCO Bacterial soap. Based on interview on 4/13/2025 at 1:00 p.m. S33 (DOH:12/17/2024) used a prohibited methodof discipline as a child was subject to physical punishment. On 4/15/2025 at 10:39 a.m., S33 pulled C1's (2-years-old) ear while on the playground with her right hand. S33 was released from employment due to the incident. This could not be corrected during the inspection. Corrective Action: Effective 5/14/2025, S1 stated she re-trained all staff on 5/15/2025 on the behavior managementpolicy to ensure compliance with this regulation.

Based on record review/interviews at 11:45 a.m., S1 and S28 failed to notify the parent immediately of the following critical incident: On 2/11/2025, at 1:10 p.m., S29 used a prohibited method of discipline. S29 pushed C1 and C2, both 1-years-old, head down on the cot during naptime. She also pulled C2s mat which could have allowed him to fall to the floor. C2s parent was not notified of the incident until 2/12/2025, at 10:26 a.m. This could not be corrected prior to the Specialist departure.
