      Based on record review at 10:30 a.m., S1 failed to have documentation that 1 of 17 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S6 failed to have the current certification. A class has been scheduled for 6/3/2025. Corrective Action: Effective 5/28/2025, S1 stated she monitor expiration dates more closely and will get all staff on same schedule to ensure compliance with this regulation.

Based on record review at 2:30 p.m., S1 failed to have documentation that S17 completed the LDE Key Training Module 1 and DCFS online mandated reporter training within 7 days of the first day present at the center and the LDE Key Orientation Training Modules 2 and 3 within 30 days of the first day present at the center:-S17's hire date and first day present at the center was 8/21/2024. Module 1 and DCFS online mandated reporter training should have been completed by 8/28/2024 and modules 2 and 3 by 9/20/2024. This was not corrected prior to the Specialist departure.
