1723.F Based on observation at 11:40 a.m., S1 failed to ensure S11 (DOH 08/01/2025) had current CPR/PFA certification approved by the Department prior to assuming sole responsibility for any children. The Specialist observed S11 supervising 6 children age three-year-olds. Corrective Action: Effective 11/05/2025, S1 stated she will ensure all new staff complete CPR/PFA certification approved by the department prior to assuming sole responsibility for any children at the center to ensure compliance with this regulation.

Based on observations at 1:30 p.m., S1 failed to have documentation that S22 (DOH 8/3/24) received orientation and completed the online Mandated reporter training within seven days of the first day present at the center and prior to having sole responsibility for any children.
