Based on observationsat 1:30 a.m., S1 failed to have documentation that S4 (First day present 4/7/2025) completed the DCFS online Mandated Reporter Training within seven days of the first day present at the center. The DCFS online Mandated Reporter Training has not been completed. Based on interviews at 11:30 a.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 5/15/2025, O1 notified S1 stating that on 5/13/2025 during pick up at 3:35 p.m., she observed S4 spraying C1, age 1 year-old, with a water bottle. 1917.A. Based on record review at 1:55 p.m., S1 failed to obtain written authorization from the parent to administer medication to C10 and C11. Corrective Action: Effective 6/9/2025, S1 stated she will have all parents complete the required documents to ensure compliance with this regulation.

1917.B.1.Based on record review at 1:55 p.m., S1 failed to ensure the prescription medication to be administered at the center was in the original pharmacy container with the complete pharmacy label. The specialist observed an Epi Pen not in original pharmacy container. Corrective Action: Effective 6/9/2025, S1 stated she will review all medication prior to acceptance from parent to ensure compliance with this regulation.

1921.E.Based on record review at 1:25 p.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March 2025, April 2025, and May 2025. Corrective Action: Effective 6/9/2025, S1 stated she will set a reminder during the months of March, April, May, and June to ensure compliance with this regulation.

1711.I. Based on records review at 11:06 a.m., S2 failed to have one additional adult present for their non-vehicular excursions. The Specialist observed Non-vehicular Excursion documentation for a Stroller Ride, on 2/25/25, which showed that 8 infants supervised by 2 Staff were present. Three staff should have been present during the excursion.
