A.8.a.b.: Based on record review and interviews at 10:34 a.m., although the center had a written Behavior Management Policy implemented, S2 (DOH 06/13/2016), used a prohibited method of discipline as C2, 2-years-old, was subject to being restrained by devices such as high chairs or feeding tables for disciplinary purposes. On 05/06/2026, at around 10:45 a.m., S2 stated to C2, 2-years-old, 'I’m about to lock you down,' and proceeded to put C2 in a high chair (booster seat with a top) for running and climbing on top of the table.       Based on observations on 05/18/2026, at 10:25 a.m., S4 failed to follow the center's Electronic Devices Policy prohibiting the use of electronic device activities for children under age two, as the Specialist observed a television in use in the infant classroom, playing a cartoon, while 4 infants were present. This was corrected at 10:26 a.m. when S4 turned off the television. Corrective Action: Effective 05/18/2026, S1 stated she will complete a daily walk-through to ensure staff are following the policy to ensure compliance with the regulation.

      1901.O.Based on record review and interviews on 05/18/2026, at 10:34 a.m. S1 failed to prohibit the possession of tobacco or unauthorized potentially toxic substances. On 5/6/2026, around 10:45 a.m., S2, had a pack of cigarettes in her pockets, while on the center premises that fell out while supervising a group of 1-2 year old children. Corrective Action: Effective 05/18/2026, S1 stated she will remind staff daily about their personal belongings and provide a space where they can keep their items to ensure compliance with the regulation.

      Based on record review and interviews on 05/18/2026 at 10:34 a.m. S1 failed to ensure the personal belongings of center staff member, S2, was not accessible to children. On 05/06/2026 at 10:45 a.m. C1 picked up S2’s green pack of cigarettes from up off the classroom floor. Corrective Action: Effective 05/18/2026, S1 stated she will remind staff daily to place their personal belongings in the closet away from children to ensure compliance with the regulation.

      Based on observations at 10:25 a.m., Specialist observed a pacifier attached, by a ribbon to the clothing of C3, 7 months. S4 removed the clip attached to the infants clothing during the licensing visit. Corrective Action: Effective 05/18/2026, S1 stated she will remind staff to not attach the infant’s pacifiers to their clothing and complete a daily walk through to ensure compliance with this regulation.
