1515.C. Based on interviews on 4/20/2024, at 2:00 p.m., S1 and S2 failed to maintain the confidentiality of C2, 1-year-old, who was involved in an incident on 4/14/2026. S1 and S2 knowingly disclosed C2âs identity, a child involved in an incident with C1, to another parent. S1 and S2 walked the parent into the classroom and identified C2 as the child that injured her child. Corrective Action: Effective 4/20/2024, S1 stated she will retrain staff on confidentiality to ensure compliance with this regulation.

1901. G.: Based on observations on 04/20/2026, at 10:00 a.m., S1 failed to ensure all of the center equipment used by children was maintained in good repair. The Specialistobserved a broken Simple 3 plastic playset, blue slide cracked open at the bottom, and two screws sticking out the walls of the playset. S1 removed the Simple 3 playset prior to Specialist leaving the center. Corrective Action: Effective 04/20/2026, S1 stated she will remove any broken yard equipment and have staff check the play yard equipment before children go outside to ensure compliance with this regulation.

Corrective Action: Effective 4/20/2026 S1 stated she will check parental notifications and documentation of all incidents and accidents at the center at the end of each day to ensure compliance with this regulation.
