Based on observation, the licensee did not comply with the section cited above. One of three playground structure slides had a hole approx. 6-7 inches long with sharp edges, which was covered with duct tape that no longer adhered. On the bridge area of the structure, there were cracks and loose areas near base of screws. It was closed off with metal gates attached with zip ties. The shade structure has loose wire rope coming down close possibly within a child's reach. This poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 08/30/2023
