Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that the floors inside of the bathroom in Room 2 had a strong odor of urine and that the floor around the toilet was starting to peel/lift up. In addition, LPA observed that the back wall and nearby floor boards in Room 2 were splattered with food stains. LPA observed that the rubber material on the stairs on the outdoors play structure was starting to crack, leaving sharp edges. POC Due Date: 06/21/2024
