      Based on record review, the licensee did not comply with the section cited abovewhich poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that the the three staff on site did not have their immunization records on file as well as a Health Screening Report. In addition, staff #1 was missing her educational record. Staff #3 was missing Mandated Reporter Training. POC Due Date: 03/10/2026
