1907.E.2. Based on observation on at 1:21 p.m., S1 failed to ensure the center's cribs were free of toys or other soft or loose bedding (including blankets). The Specialists observed C3 , age 10-months-old, asleep in his cribs with a loose blanket covering him. This was corrected at 1:22 p.m. when S11 removed the blanket from S3âs crib. Corrective Action: Effective 04/15/2026, S1 stated she will conduct walkthroughs to ensure infants that are placed in their cribs are free of blankets to ensure compliance with this regulation.

1909.C. Based on observations and record review at 9:44 a.m., S1 failed to ensure a written authorization from a physician was available for infants to be allowed to sleep in bouncers. The Specialists observed C1, age 7-month-old, and C2, age 11-month-old, C3, age, 10-month-old, sleeping in a bouncer. This was corrected at 9:58 a.m. when S11 moved the infants to a crib. Corrective Action: Effective 04/15/2026, S1 stated all infants will be moved to a crib once they fall asleep to ensure compliance with this regulation.

1919.A.&B. Based on observations at 11:35 a.m., S1 failed to have menu substitutions or additions posted, written or electronically, on or near the menu. The Specialist observed the menu listed smothered chicken, rice, string beans, sliced peaches, bread, and milk; however, the children were served barbecue pork chops, mashed potatoes, corn, apple sauce and milk. Corrective Action: Effective 04/15/2026, S1 stated she will ensure the menu is up-to-date first thing in the morning to ensure compliance with this regulation.
