C.: Based on record review/interview(s) at 10:00 a.m. on 2/10/2025, S1 failed to notify Child Welfare within 24 hours of the following critical incident: On 2/5/2025 at 12:02 p.m., S12 was observed on the center's camera picking C1 (four-years-old) up aggressively and moving him to the opposite side of the classroom while holding one of his arms which resulted in several scratches on both of his arms. The incident occurred on 2/5/2025, but it was not reported to Child Welfare. S1 stated she did not report the incident to Child Welfare because the Sheriff's office informed her they…

Based on observations/interviews at 10:00 a.m., although the provider has a behavior management policy that prohibits corporal and physical punishment. S12, (DOH: 8/19/2024), used a prohibited method of discipline on 2/5/2025 at 12:02 p.m. Specialist reviewed video footage of the 3-4 year-old classroom on 2/5/2025 and observed S12 pick C1 up aggressively and pull him to the opposite side of the classroom holding one of his arms which resulted in several scratches on both of his arms. S12 was terminated on 2/5/2025.

Based on observations on 2/10/2025, S12 failed to use proper lifting techniques when she picked C1 up from the time out area on 2/5/2025 as she lifted C1 from the floor using both of his arms.
