1901.M. Based on observations at 10:35 a.m., S1 failed to ensure strings and cords were not accessible to children under age 4. Specialists observed a computer cord hanging from a box inside a bookshelf in classroom # 2. S removed the computer cord before the specialists left out of the room. Corrective Action: Effective 11/20/2025, S1 stated she will complete daily walkthroughs of the classrooms ensure compliance with this regulation.

1903.E. Based on observations at 10:50 a.m., S1 failed to ensure the outdoor play space had an enclosure with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The Specialists observed 1 of 2 gates to be nonfunctioning allowing it to be pushed open. Corrective Action: Effective 11/20/2025, S1 stated she will place a lock on the side to ensure compliance with this regulation.

1911.J. Based on observations at 10:33 a.m. and 10:36 a.m., S1 failed to ensure S3 practiced proper lifting of a child. The Specialists observed S3 lift C2, age one-year-old, by his right arm, to move him from the door entrance and at 10:36 a.m. to remove him from sitting in from of a swing. Corrective Action: Effective 11/20/2025 ,S1 stated she will complete daily walkthroughs to ensure compliance with his regulation.

1919.H. Based on observations at 9:31a.m., S4 failed to hold C1, age ten-months-old while being bottle-fed. While viewing daily infant reports in Procare, S4 verified to the Specialists what time C1 had a bottle by showing a picture, sent to the parent at 9:31 a.m., that showed C1 holding her bottle while in a baby bouncer seat. Corrective Action: Effective 11/20/2025 ,S1 stated she will retrain all staff on properly feeding of infants to ensure compliance with his regulation.

1919.J. Based on observations at 10:40 a.m., S1 failed to ensure all bottles were labeled with the each child's name. The Specialists observed several unlabeled bottles in the center’s refrigerator that contained substances for children in care. Corrective Action: Effective 11/20/2025 S1 stated she will mark all bottles with the children’s name to ensure compliance with this regulation.

713 A. Based off of record review at 10:30 a.m., S1 failed have to have an updated City Fire Inspection from the past twelve months on file. The certificate present at the center expiration date listed is 6/30/2025. Corrective Action: Effective 11/20/2025, S1 stated she will complete the required City Fire inspection and maintain require for review to ensure compliance with this regulation.

C. Based on record review/interviews at 1:53 p.m., S1 failed to notify DCFS within 24 hours of the following critical incident: On 5/15/2025 C1, age two -years-old, walked out of the centers side exit door. C1 was brought back into the center by O2, a relative, who was picking her up for the day. S2 stated she contacted DCFS via phone but did not have any documentation to provide the Department. 1713.A.B.C. Based on interviews at 12:53 p.m. 2:15 p.m. S4, S5, and S6 failed to ensure children were under supervision at all times. On 5/15/2025 between 10:15 10:20 a.m., C1, age two -years-old, walked out of the centers side exit door. C1 was brought back into the center by O2, a relative, who was picking her up for the day. According to S8, she opened the side door after she heard a knocking. C1 was with O2. O2 stated C1 was standing by the gate. 1719.B.C Based on observations at 12:00 p.m., S1 failed to have documentation that S4 completed the LDE Key Training Module 1, within seven days of the first day present and LDE Key Training Module 2 and 3 within 30 calendar days at the center and prior to assuming sole responsibility for any children. S4s first day present at the center 4/22/25. Corrective Action: Effective 5/28/25, S2 stated she will ensure all new staffs completed all required trainings within the 7 days of hire date and to ensure compliance with this regulation

1711.A.B. Based on observation at 9:11 a.m., S2 failed to meet the required child to staff ratio for infants. The specialist observed S3, alone, with 6 children (ages 11 months to one-year-olds). The required ratio for children of this age is 5 children per 1 staff person. One additional staff was needed to satisfy child to staff ratio. S2 corrected child to staff ratio at 9:12 a.m. by placing S4 in the classroom.
