Based on record review at 11:30 a.m., S1 failed to notify LDOE and DCFS within 24 hours of the following critical incident: On 11/21/2024 at 5:03 p.m., O1, C1's mother, notified the center, through Brightwheel, alleging S14 slammed C1's, age - one-year old, head against the changing table. S1 submitted a critical incident report to LDOE on 11/25/2024, but failed to report the allegation to DCFS.
