The Specialists observed 5 children, age 1-year-old, unsupervised napping in Classroom 3. S6 was in classroom 1 with a 1-year-old trying to put him to sleep. Supervision was corrected at 1:10 p.m., when S6 walked back into the classroom. It is unknown how long the children were alone. 1713.A.B.C.       1711.A.B.D.E. Based on observations on 7/17/2025, at 9:45 a.m., S7, S8, and S9 failed to meet the required child to staff ratio for infants. The Specialist walked into the second building at the same time as S8 and S9. The Specialist observed S7 caring for 9 children, ages 6-months to 1-year-old. The required ratio for infants under one-year-old is 5 children per 1 staff person. One additional staff was needed to meet ratio. Based on records reviewed on 7/17/2025, at 11:30 a.m., S1 failed to have documentation that S12 and S14 completed the annual Online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. S12's training expired on 1/3/2025, and S14's training expired on 1/4/2025. This was not corrected during the inspection. Corrective Action: Effective 7/17/25, S1 stated she will set a reminder 15 days prior to all staffs certification expiration date and all certifications sent to the office upon completion to ensure compliance with this regulation.

Based on record review/interviews at 2:21 p.m., S1 failed to notify DCFS within 24 hours of the following critical incident: On 2/17/2025, at approximately 11:00 a.m., O1 informed S1 that he witnessed S6 yell and grab C1, age three-years-old, and threw him to the ground. S1 notified the parent of C1 on 2/17/2025. LDOE was notified on 2/17/2025. S1 did not notify Child Welfare. Child Welfare visited the center the week of 2/17/2026 to investigate the incident. The Specialist could not validate the allegation.

Based on record review/interviews at 2:37 p.m., S1 failed to have documentation of an accurate daily visitors attendance record that included the first and last name, date of visit, arrival and departure times, and purpose of the visit. S1 stated DCFS came to the center during the week of 2/17/2025 to conduct an investigation regarding C1, age three-year-old.

      o1711.A.B.DG. Based on observation at 1:49 p.m., S1 failed to meet the required child-to-staff ratio for infants. The Specialists observed S7, alone, with 6 infants, ages 4 months to one-year-old. The required ratio for infants is five children per 1 staff person. One additional staff was needed to satisfy child to staff ratio. The ratio was corrected at 2:03 p.m. when S4 entered the classroom.

Based on record review/observations at 1:27 p.m., children failed to be under supervision at all times. -The Specialists observed 11 children napping, age three-years-old, left alone, unsupervised in Classroom 6. -The Specialists observed S4 walk out of Classroom 3 leaving 8 children napping, age two-years-old, alone, unsupervised. Supervision was met at 1:34 p.m. when S4 walked in the classroom and combined Classroom 3 and 6.
