Based on record review at 10:55 a.m., S1 failed to have documentation that 3 of 9 staff, completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS. S1 and S2s training Mandated Reporter Training expired on 06/30/2025. S7s Mandated Reporter Training expired on 12/5/2024. Corrective Action: Effective 08/19/2025, S1 stated he will set a reminder to have staff complete the required training yearly to ensure compliance with this regulation.

      1911.G. Based on observations at 9:30 a.m., the Specialist observed a pacifier attached, by a clip, to the clothing of C1, age 6-months-old. S5 removed the clip attached to the infants bib during the licensing visit. Corrective Action: Effective 08/19/2025, S1 stated he will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.
