Based on observation at 11:40 a.m., the manufacturer's restraint device failed to be in use when a floor seat was occupied by a child in S15's classroom. The Specialist observed C4, 2-year-old, seated in the floor seat and the manufacturer's restraint device failed to be in use. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 11/4/2025, S1 will review with all staff that the manufacturer's restraint device must be engaged at all times when children are present in the floor seat to ensure compliance with this regulation.
