      Based on observations on 03/30/2026, at 12:30 p.m., S1 failed to ensure that no electronic devices are used for children under age two-years-old. The Specialist observed the television turned on for viewing in the infant classroom, in which S5 was caring for 4 children, aged 5-8 months-old. Corrective Action: Effective 03/30/2026, S1 stated she will retrain all staff on the center's electronic device policy to ensure compliance with this regulation.

    • CPR and First Aid Certifications1723.F

      Based on observations, record review, and interview on 03/30/2026, S4 failed to have current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. The Specialist observed S4 working alone in the classroom with 8 one-year-old children during naptime. Corrective Action: Effective 3/30/2026, S1 stated she will schedule a CPR class for S4 immediately to ensure compliance with this regulation.

      Based on record review and interview on 03/30/2026, S1 failed to provide documentation that 10 of 11 staff, S1, S2, S3, S4, S6, S7, S8, S9, S10, and S11, completed the online child abuse and neglect Mandated Reporter Training provided by DCFS annually as required. Corrective Action: Effective 03/30/2026, S1 stated she will have staff complete the Mandated Reporter training immediately to ensure compliance with this regulation.

      Based on record review/interview on 12/18/2024, S1 failed to maintain an updated CCCBC-based determination of eligibility for each staff member employed at the center. S4 (DOH 5/13/2019) CCCBC's eligibility expired on 04/22/2019. S1 stated S4 has not been consistently working at the center and acts a substitute.
