On 4/16/2026, at approximately 8:32 a.m., S9 and S10 were in the classroom supervising 15 children, ages 3 to 5 years old. S10 accidentally served breakfast with regular milk to C1 (4 year-old), instead of lactose-free milk, as documented on file and in the classroom on the covered allergies list, located on the wall.
