      Based on record review/interviews at 10:15 a.m., S10 failed to serve the proper foods and drinks identified by the parent to a child with allergies or special diets. On 4/16/2026, at approximately 8:32 a.m., S9 and S10 were in the classroom supervising 15 children, ages 3 to 5 years old. S10 accidentally served breakfast with regular milk to C1 (4 year-old), instead of lactose-free milk, as documented on file and in the classroom on the covered allergies list, located on the wall.       Based on record review/interview at 9:15 a.m., on 1/28/2026, S9 and S10 failed to immediately notify the Parent of the following critical incident: on 1/23/2026, at approximately 9:57 a.m. C1, four-year-old, was washing hands at the classroom sink, then C2, four-year-old, was standing behind C1 and touched her buttocks and between her legs inappropriately.
