C.: Based on record review at 3:00 p.m., S1 failed to ensure that all staff have completed the required minimum of 12 clock hours of continuing education each anniversary year with trainers approved by the department. S3 had 7.5 hours, S4 had 6 hours, S5 had 9 hours, S6 had 10.5 hours, S8 had 6 hours, S13 had 1.5 hours, S14 had 1.5 hours and S15 had 10.5 hours. This could not be corrected prior to the Specialist departure.
