      Based on observations at 10:30 a.m., the number of children using Room 3 was exceeded based on the 35 square feet per child requirement. Room 3 can accommodate 11 children and 14 were present. This was corrected prior to the Specialist exiting the center. Corrective Action: Effective 1/13/2026, S1 will create a room capacity sign to ensure compliance with this regulation.

The center is licensed to provide care for children ages 2-years-old through 5-years-old. The Specialist observed 4 infants aged, 5-months-old to 8-month-old, present in S5's classroom.

Based on review of records at 1:30 p.m., the daily attendance log for children failed to include the timeof arrival and departure of each child and the name of the person to whom the child was released. The Specialist reviewed records from 1/6/2025 until 1/31/2025, and verified the following: the arrival time failed to be documented 2 times, the departure time failed to be documented 5 times; and the first and last name of the person to whom the child was released failed to be documented 12 times.
