1727.A. Based on record review/Interview at 12:15 p.m., S1 failed to provide documentation that all staff completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. The Specialist observed that 17 out of 18 staff had Mandated Reporter training. S15 did not have Mandated Reporter training. S15's Mandated Reporter training expired May 2025. Corrective Action: Effective 6/12/2025, S1 stated S15 will complete training on today, to ensure compliance with this regulation.

Based on Record Review/Interview at 11:00 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed that the End-of Day-Check was not completed for March 17-19, 21, 24-26, and 28 of 2025, April 1-4, 8-10, and 29-30 of 2025 and May 1-2, 15-16, 26, and 28-30 of 2025. Corrective Action: Effective 6/12/2025, S1 stated she will search for missing files and check daily in the evenings before closure, to ensure compliance with this regulation.

Based on observations at 9:35 a.m., S4 and S5 failed to ensure centers cribs were free of toys or other soft or loose bedding (including comforters, blankets, sheets, bumper pads, pillows, stuffed animals, and wedges. The Specialist observed 3 infants sleeping in their individual cribs with loose bedding. The infant room had a total of 10 cribs with loose bedding. Corrective Action: Effective 6/12/2025, S1 stated an order has been placed for firm fitted sheets, to ensure compliance with this regulation.
