1713.A.B. Based on interviews on 4/23/2026, at 12:00 p.m., S4 failed to ensure children were supervised at all times. On 3/31/2026, while S4 was changing a child on the changing table C1 and C2, ages 2 years old left out and walked to the right towards the front outside of the center. She immediately messaged the front desk for assistance, but no one has responded. She took the 8 remaining two-year-old children to S6 and S7âs classroom. 1903.C. Based on observation and interviews on 4/23/2026, at 10:00 a.m., S1 failed to ensure the center was free of hazards. In the Bear Classroom, the Specialists observed a broken door in the back of the classroom. The latch on the door was not working properly. The door could easily be pushed open. The door was being blocked by a baby gate preventing the children from leaving out of the door. Corrective Action: Effective 04/23/2026, S1 stated she will repair the lock in the classroom to ensure compliance with this regulation.

Based on record review and interviews at 11:00 a.m., S1 failed to notify the parent immediately and the Department and Child Welfare within 24 hours of the following critical incident: On 12/4/2025, a parent notified S21 of an alleged incident that occurred on 11/10/2025 around 3:43 p.m. C1, 2-year-old, attempted to bite a classmate. S20 removed C1 from the incident, placed him in the corner and bit him.
