Based on record review and interviews at 9:30 a.m., S1 failed to have documentation that all staff members on the premises and accessible to children have current certification in infant and child CPR and adult CPR through training approved by the department. S3 did not have this certification. Corrective Action: Effective 2/13/2026, S1 stated she will schedule a class for S3 by the end of today and have her certification within 30 days to ensure compliance with this regulation.

S3 did not have this certification. Based on observations/interview at 12:00 p.m., S1 failed to ensure that no electronic devices are used for children under 2 years old. The Specialists observed four infants, aged 5 months old to 1-years-old, watching television while in cribs and bouncers. Corrective Action: S1 stated that she will turn off and unplug the televisions in the infant classroom and ensure they remain off. S1 will continue to monitor each classroom to ensure that this regulation is met at all times.

: Based on record review and interview at 12:00 p.m., S1 failed to ensure that each child present at the center contained a childrens cumulative file. During a walkthrough of the center, the Specialist observed C2, four-years-old, in the classroom with S3. S1 stated the child was her niece and was a drop in at the center. When the Specialist requested a Childrens Cumulative File for C2, none was present. Based on observations at 12:00 p.m., S1 failed to meet the required child to staff ratio for children in the center. The Specialists observed (9) one-two-year-olds in the classroom with S2. The maximum required ratio for children that are one-year-olds is 7 children per 1 staff member. S2 removed C3 from the classroom and placed him in the infant classroom with S1 and ratio was met. : Based on interviews at 12:00 p.m. S1 failed to ensure that children were supervised at all times. Staff interviews indicated that due to low staff numbers, the centers current staff are required to supervise multiple classrooms at one time so that other tasks around the center may occur such as fixing lunch, leaving other classrooms with no assigned staff member at all times. Based on observations at 12:00 p.m., S1 failed to ensure that the all cribs were free of toys and other soft or loose bedding, including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges when a child is asleep in the crib. The Specialist observed C1, six-months-old, laying on an U-shaped boppy pillow inside of the crib with the television on falling asleep. S1 removed the boppy pillow from the crib and turned off the television prior to the Specialist leaving the classroom and this regulation was met.…

Based on record review/interview at 12:00 p.m., S1 failed to ensure that the center has daily written or electronic reports for infants in attendance at the center. During the walkthrough of the center, no Daily Infant Reports were present. S1 stated that she was unable to complete the logs due to being busy in the center. Corrective Action: Effective 06/27/2025, S1 stated she will ensure Daily Infant Reports are completed, and continue to monitor the Daily Reports for Infants to ensure they are being completed daily to ensure compliance with this regulation.

Based on observations at 12:00 p.m. S1 failed to ensure all bottle formula and breast milk were labeled with the childs name. The Specialist observed 2 bottles in the infant classroom unlabeled. Corrective Action: Effective 06/27/2025, S5 stated she will monitor all bottles in the infant classroom and ensure all bottles are labeled with the childs first and last name to ensure compliance with this regulation.
