Based on observations and interview at 12:00 p.m., S1 failed to ensure that all strings and cords were inaccessible to children. The Specialist observed several electronic cords tangled together and plugged up under a bookcase in the 3-year-old classroom, accessible to children. Before the Specialist departed the center, all cords were picked up, and removed from the classroom. On or around 12/19/2025, C2, two-years-old, injured her nose while outside on the playground, leaving a bruise on the bridge of her nose.
