Based on record review and interview at 10:45 a.m., S1 failed to maintain documentation of a daily attendance record for staff to include their time of arrival. The Specialist observed S2 present at the center, but she was not signed in on the attendance log. S1 stated S2 arrived at the center at 7:00 a.m. Corrective Action: Effective 03/18/2026, S1 stated she will review the staff attendance log with the employees by 03/20/2026, to ensure compliance with this regulation.

1713.A.&B.&C.: Based on observation at 10:30 a.m., S1 failed to have C1, two-years-old, under supervision at all times. The Specialist observed C1 in another room napping as S1 and S2 were supervising the other children finishing lunch. S1 immediately took C1 into the room with them upon the Specialist's request. Corrective Action: Effective 03/18/2026, S1 stated she will ensure no child is in left in a different room unsupervised, and meet with staff to review supervision by 03/20/2026, to ensure compliance with this regulation.
