An employment history verification was not documented and on file. CCF Handbook, Section 5.1, D (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] During review of staff files licensing specialist observed a new staff member file did not have employment history documented. Provider must have a five-year employment check by 6/16/2026. 5 Background Screening 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. Due Date 6/16/2026

Children in the outdoor play area were not adequately supervised in that [staff member was not engaging or supervising the children.]. CCF Handbook, Section 2.4.1, A General Requirements [SR] Staff and another member of this department reviewed playground cameras and observed a staff member not engaging of supervising children in care. The staff member was let go on 3/20/2026 after an internal investigation. 2.4 Supervision 2.4.1 General Supervision Requirements A. The nap/sleep space was under furniture or against furniture that created a hazard. CCF Handbook, Section 3.6.2, D Physical Environment [SR] Licensing specialist observed a child sleeping under a wall unit anchored on wall used as cubbies and coat hanger. Provider moved the child from under the wall unit to another safe space in the room at time of inspection. 3.6.2 Nap/Sleep Space D. Napping and sleeping spaces must not be under furniture or against furniture that creates a hazard. Due Date Completed at time of inspection

Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Enforcement [SR] Staff member used her knee to move the child in the direction she desired while telling the child to move.The teacher was terminated on 2/5/2025. The child is still in attendance. 8.2 Child Safety C. Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited.

Supervision of children in the [two-year-old ] group was inadequate in that [a child was left on the playground]. CCF Handbook, Section 2.4 General Requirements [SR] A child in the two-year-old group was left on the playground unattended for approximately 20 minutes. The child was found unharmed sitting by a bench next to the door. This provider going forward will have two staff members do a face- to - face count every time during any outdoor or transition period. A third count will be done by the supervisor. 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow child care personnel to see and supervise children while in care. E. During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care [during the month of January 2024]. CCF Handbook, Section 3.8.4, A Physical Environment [SR] In reviewing fire drill logs, counselor observed that there was no fire drill conducted for the month of January 2024. A fire drill was conducted in the presence of this counselor during inspection. There were 73 children and 13 adults who participated on 2/6/2024. This was then documented by the director.

Inspections were not conducted and documented monthly of all supports, above and below the ground, all connectors, and moving parts. CCF Handbook, Section 3.12, B During inspection counselor observed that Physical environment inspections were not conducted or recorded since September 2023. Counselor inspected the playground to be in good condition and provider recorded the inspection during inspection 2/6/2024.

The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Record Keeping [SR] In reviewing personal files counselor observed two staff members did not have an updated abuse and neglect document. This is due to be completed and submitted to this counselor by 2/13/2024. 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.

Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Record Keeping [SR] During inspection counselor observed there was a maintenance person in the building hired by Kinder care and works throughout all kinder cares in Florida. This provider did not have a file for this counselor to review. Background screening was pulled and counselor found that this person had expired fingerprints since 3/18/2023. This staff was let go immediately until Background screening becomes eligible. 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file.
