Breastmilk or formula was fed to the wrong child. CCF Handbook, Section 3.9.6, C Food and Nutrition [SR] On 12/16/25, Director self-reported that an infant was given another infant's bottle with breast milk. Bottle was labeled however staff member assumed that this bottle was for the infant she was holding. Staff member understood and acknowledged that she failed to read label of bottle. Director reviewed and retrained staff member in always verifying the names on the bottles to match infants being fed. 3.9.6 Breastmilk, Infant Formula, and Food page 35 C. Breastmilk or infant formula provided for a specific infant by a parent or guardian should not be fed to other children. In the event that the wrong breastmilk or formula is given to an infant in care, the provider must immediately inform the childs parent or legal guardian of the incident, as well as the parent or legal guardian of the infant that the formula or breastmilk was intended for. The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 (Section 2.1 Health and Safety, Page 3) Training [SR] One staff member, start date 7/15/24. signed up on 7/19/24 for safe sleep practices ad shaken baby syndrome however did not complete training as required within 30 days. 4.2 Training Requirements page 40 and 41 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Department's training transcript in the child care personnel record.

Child care personnel did not complete preservice training requirements prior to unsupervised contact with or care of children or, if supervised, within 90 days of beginning employment or service with the provider. (Section 3.1 and 3.2 Training Requirements, Page 7) [SR] One staff member signed up for required School Readiness Training on 6/29/24 but did not complete training in required timeframe. School Readiness Program Health and Safety Standards Handbook 18 Training Requirements page 41 18.1 Pre-service Timeframe All pre-service training requirements listed below must be completed by all program personnel, volunteers and substitutes, each as defined in this handbook, within 90 days of initial employment with any provider participating in the school readiness program. This timeframe does not start over if personnel change employment to another school readiness provider within this 90 days. Personnel who have not completed all pre-service training requirements may not be allowed any unsupervised contact with or care of children in a school readiness program.

The director responsible for the daily operation of the program did not have an active Director Credential. CCF Handbook, Section 4.7 The facility lost its Director on 2/13/25. A new Director meeting qualification will need to be hired by 8/13/25 to be in compliance. A Provisional License will be issued until compliance can be achieved. 4.7 Director Credential pages 46 and 47 Section 402.305(2)(g), F.S., requires a childcare facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a childcare facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CF-FSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C-22.001(7)(k), F.A.C. CF- www.myflfamilies.com/childcare. All applications and documentation will be verified, and, if complete, the credential will be issued by the Department or its designated representative on CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, which is incorporated by reference in 65C-22.001(7)(g), F.A.C. A. Each childcare facility must have a credentialed director who is on-site a majority of hours, excluding weekends and evening hours that the facility is in operation. Documentation of majority of hours must be maintained and available for review by the licensing authority.

Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B Food and Nutrition [SR] Counselor observed water bottles not all labeled with children's first and last name. This was corrected at this visit. 3.9 Food Preparation/Food Service page 34 and 35 3.9.5 Food Handling B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily.

The facility failed to maintain a current Employee/Contractor Roster for all child care personnel in the Clearinghouse. CCF Handbook, Section 5.1, K Counselor observed BGS roster as it was not updated for old or new staff within time frame. Corrections were made to roster at this visit 7/29/24. 5 Background Screening page 51-52 5.1 Initial Screening K. The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 5 days of when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 5 days of the employment termination.

Supervision of children in the [2 year old ] group was inadequate in that [1 child was left unattended in classroom]. CCF Handbook, Section 2.4 General Requirements [SR] Director self-reported that a child was left in the classroom when the caregiver transitioned children to outside play space. Caregiver is no longer employed with facility and child is no longer enrolled or in attendance. 2.4 Supervision page 12 2.4.1 General Supervision Requirements B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location.
