Based on record review and interviews at 10:45 a.m., S1 failed to ensure information concerning a child or his/her family, directly, or indirectly, was not disclosed or knowingly permitted the disclosure to an unauthorized person. S1 disclosed C1's date of birth, parent's name, and parent's phone number to O1. This could not be corrected. Corrective Action: Effective 5/28/2026, S1 stated she will not give out any confidential information to unauthorized persons and without the parent's written consent to ensure compliance with this regulation.

Based on interviews at 1:30 p.m., S1 lacked documentation of an information form that includes the person's name, address, phone number, list of duties performed while present at the center and/or documentation of a CCCBC-based determination of eligibility for O2, O3, O4, O5, O6, O7, O8, O9, O10, O11, and O12. This could not be corrected. Corrective Action: Effective 5/28/2026, S1 stated she will have all contractors complete the independent contractor information form upon their arrival to ensure compliance with this regulation.

Based on record review at 1:30 p.m., S1 failed to have documentation 2 of 15 staff on the premises and accessible to children have current certification in infant, child, and adult CPR and Pediatric First Aid through training approved by the Department. S8 (first day working 3/11/2026) failed to have the current certification and failed to be supervised as she was observed supervising four 1-year-old children and S6 (first day working 3/16/2026) was observed supervising fourteen 5-8-year-old children alone. Based on record review and interview at 1:30 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the department for O2, O3, O4, O5, O6, O7, O8, O9, O10, O11 and O12, independent contractors, prior to them being present at the center or performing services. S1 lacked documentation of the paid, adult staff member not otherwise counted in child to staff ratios who accompanied the independent contractors at all times while on the center premises. Based on observations at 10:10 a.m., S4 and S5 failed to ensure the center's cribs were free of soft or loose bedding (including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges) while the child was in the crib. The Specialist observed C3, 7-months-old in his crib with a boppy. This was corrected when the boppy was removed. Corrective Action: Effective 5/28/2026, S1 stated she has already spoken with the teachers to instruct them to allow children to be placed on the boppies on the floor and not in the cribs to ensure compliance with this regulation.

Based on observations at 10:10 a.m., S4 and S5 failed to ensure documentation was available for 2 of 9 infants as required for infants to sleep in a car seat or other similar device. The Specialist observed C2, 3-months-old and C4, 5-months-old, asleep in swings. This was corrected when they were moved to cribs. Corrective Action: Effective 5/28/2026, S1 stated she has already spoken to the teachers and instructed them to immediately move children who have fallen asleep to a crib to ensure compliance with this regulation.

Based on observations at 10:10 a.m., S4 and S5 failed to ensure no bibs were worn by any child while they were asleep. The Specialist observed C2, 3-months-old, and C3, 7-months-old, asleep with their bibs on. This was corrected. Corrective Action: Effective 5/28/2026, S1 stated she has already instructed the teachers to remove all bibs before putting children in their cribs to ensure compliance with this regulation.

Based on record review and interviews during the walkthrough, S10, S11, S6 and S16 failed to ensure daily observations were completed upon children's arrival to the center for 5/28/2026. This could not be corrected. Corrective Action: Effective 5/28/2026, S1 stated she and/or S3 will do morning checks around 9:30 a.m. to ensure staff are completing the daily observations and to ensure compliance with this regulation.

Based on record review at 1:30 p.m., S1 failed to ensure documentation of the visual checks of the vehicles were complete as they did not include the time the vehicle was checked on 4/27/2026-4/28/2026, 5/4/2026, 5/12/2026 and 5/14/2026. This could not be corrected. Corrective Action: Effective 5/28/2026, S1 stated she and S3 will check the logs after the routes to ensure they are completed correctly and to ensure compliance with this regulation.

Based on record review/interview at 2:00 p.m., although S16 changed the diaper of C4, 2-years-old, when soiled, on 9/3/2025 and 9/4/2025, S16 failed to ensure C4 was cleaned as there were traces of feces left on the child’s leg and shirt. This could not be corrected. Corrective Action: Effective 2/10/2026, S1 stated she has already spoken to the staff. The staff have to document children ages 0 - 1-years-old's diaper changes in Brightwheel to ensure compliance with this regulation.
