S7s hire date and first day present at the center was on 03/20/2025. Module 1 and DCFS mandated reporter training should have been completed by 03/27/2025. This was not corrected prior to the Specialists departure.

The Specialist observed S7 (DOH: 03/20/2025) supervising ten 2-year-olds, alone, and S12 (DOH: 12/02/2024) should have received certification in pediatric first aid and CPR by 03/02/2025. This was not corrected prior to the Specialists departure.
