      1719.A.B.&C Based on record review at 12:15 p.m., S1 failed to have documentation that S6 received center-specific orientation and completed the DCFS online Mandated Reporter training within seven days of the first day present at the center. S6's first day present at the center was 05/04/2026, the DCFS online Mandated Reporter training has not been completed. Corrective Action: Effective 05/20/2026, S1 stated all new staff complete center-specific orientation and the DCFS online Mandated Reporter training within 5 days of the first day present at the center.

      1711.A.B.D.E. Based on observations at 10:45 a.m., S1 failed to meet the required child to staff ratio for infants. The specialist observed S1 caring for 11 children, ages eight-months to three-years-old, from 10:49 a.m. to 11:06 a.m., while S2 prepared lunch for all of the children. The required ratio for infants under one-year-old children is 5 children per 1 staff person. One additional staff was needed to meet ratio. Ratio was met at 11:06 when S2 finished serving lunch. There were two infants, two - one-year olds, one - two-year old, and six three- year olds.

      C. Based on records review at 10:55 a.m., S1 (DOH 9/24/20) failed to have current documentation of training in medication administration completed with an approved Child Care Health Consultant. Training is required every two years and the staff's training expired 8/20/24.

There failed to be results including an explanation from parent and/or child documented. S1 failed to continue recording observations for the following dates 12/13/24, 12/16/24, 12/17/24, 12/18/24, 12/19/24, 12/21/24, 12/27/24, 12/30/24, 1/3/25, and 1/6/25.
