Based on interviews at 10:00 a.m., on 4/28/2025 at approximately 4:47 p.m., S47 released C1, 4-years-old, to a person not authorized by the parent. Corrective Action: Effective 5/7/2025, S9 stated she will update the signed release forms at the center, office quarterly and on the bus to ensure compliance with this regulation.

Based on interviews at 10:00 a.m., S47 failed to maintain signed parental authorization to transport C1, 4-years-old in her personal vehicle on 4/28/2025. The authorization shall include the name of the child, the type of transportation (to and from school, to and from home) and the names of the individuals, schools, or entities to whom the child may be released. Corrective Action: Effective 5/7/2025, S9 stated she make sure the parents come pick the children up or the staff get the authorization prior to ensure compliance with this regulation.
