S1, S2, S3, S4, S6, and S7 failed to have documentation of completing the online Mandated Reporter Training annually. S1 and S3's last training expired on 7/23/2025. Based on observation at 11:15 a.m., S2 failed to ensure the manufacturer's restraint device was used when occupied by children. A 3-month-old and 10-month-old were not properly restrained in bouncers. This was corrected prior to the Specialist departure. Corrective Action: Effective 7/30/2025, S1 stated she will re-train the staff on restraints to ensure compliance with this regulations.

Based on observation at 11:15 a.m., S2 failed to have written authorization from a physician required for a child to sleep in a car seat or other similar device. A 10-month-old was asleep in a bouncer. This was corrected prior to the Specialist departure. Corrective Action: Effective 7/30/2025, S1 stated she will have a staff meeting with staff and discuss car seats and infant safety to ensure compliance with this regulation.

Based on observation at 11:15 a.m., S2 failed to ensure bibs were not worn by a child while asleep. A 10-months-old was asleep in the crib with a bib on. This was corrected prior to the Specialist departure. Corrective Action: Effective 7/30/2025, S1 stated she will have a meeting with staff to discuss infant safety to ensure compliance with this regulation.

Based on record review/interview at 11:15 a.m., S1 failed to have daily written or electronic reports for 4 infants ages, one 3-month-old, three 10-month-olds. S2 stated she hadn't had time to start the reports, but would do so. S2 provided 3 reports to the Specialist that were already completed for the rest of the day. Based on observations/record review at 12:30 p.m., S1 failed to ensure the number of children using a room was based on the 35 square feet per child requirement. The Specialist observed S7 and S9 supervising S15 children, aged 2-years-old, in Classroom 2. Based on the center's approved floor plan, the room capacity for Classroom 2 is 7 children. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/22/2025, S1 stated she will make the adjustments based on the floorplan and attendance to ensure compliance with this regulation.

Based on observations at 9:30 a.m., on 03/19/2025, S5 failed to meet child-to-staff ratio as the Specialist observed S5 supervising 15 children aged 2-years-old, alone. S5 could only supervise 10 children. There needed to be 1 additional staff in the classroom. This was not corrected prior to the Specialist's departure.
