Based on observation and record review at 11:15 a.m., S1 failed to ensure that 1 of 10 staff, S2, on the premises and accessible to the children, has a current certification in infant, child, and adult CPR through training approved by the Department. S2's skills review is scheduled for 9/23/2025. Based on observation and record review at 11:15 a.m., S1 failed to have documentation of S17's CCCBC-Based Determination of Eligibility available on the centers roster at all times. This was corrected during the inspection. Corrective Action: Effective 9/17/2025, S1 stated she will reach out to the company's main office to add all new hire staff prior to their first day working. S1's email will be added to the CCCBC system to gain access to the center's roster at all times, to ensure compliance with this regulation.

Based on record review at 2 p.m., S1 failed to have documentation on file for Independent Contractors, O1-O3, that included their name, address, phone number, list of duties performed while at the center. S1 failed to have documentation of a CCCBC-based determination of eligibility or the name of the adult staff member not counted in ratio who accompanied O2 on 7/1/2025 and 7/22/2025. Based on record review at 11 a.m., S1 failed to ensure 1 of 6 staff, S4, received the required DCFS online Mandated Reporter Training within 7 days the of first day present at the early learning center on 7/10/2025. S4 completed this training on 7/20/2025 and was due to complete this training no later than 7/16/2025. This could not be corrected.
