      Based on record review and interviews on 10/3/2025, S1 failed to ensure notification was made to the Department and Child Welfare within 24 hours of the following critical incident: On 9/23/2025, at 5:06 a.m., S6 was notified by O2, via text message, that C1 stated S2 hit her on her hand. S1 notified the Department on 9/26/2025. The Specialist was unable to validate the allegation.       Based on observation on 9/26/2025, at 10:37 a.m., the number of children using the infant classroom exceeded the 35 square feet per child requirement. The classroom can accommodate 9 children, and 10 children were present. This was not corrected during the inspection, and was observed corrected on 10/3/2025, Corrective Action: Effective 10/3/2025, S1 stated she reviewed the approved classroom capacity with the infant staff and will further review all classroom capacities with all the staff, to ensure compliance with this regulation.
