Based on observations/interviews on 5/22/2025 at 9:15 a.m., S3 and S4 failed to follow the Electronic Devices Policy. The Specialist observed television programming displayed in an infant classroom. Corrective Action: Effective 5/22/2025, S1 stated she will enforce closing of laptops in the classrooms allowing only music to play via bluetooth, to ensure compliance with this regulation.

Based on observations, on 5/22/2025, at 9:00 a.m.., S1 and S3 failed to maintain close proximity or accompany children while they were walking to the classroom. The Specialist observed one, four-year-old, (C1), wandering in the hallway near the front office, out of S1 and S3s sight. S1 and S3 did not provide adequate supervision for C1 while the child walked to the front of the center. Corrective Action: Effective 5/22/2025, S1 stated she and staff will accompany children at all times when walking through center, to ensure compliance with this regulation.

Items That Can be Harmful to Children- Based on observation/ interview on 5/22/2025 at 10:00 a.m., S1 failed to ensure items that can be harmful to children, such as: crawlspaces and mechanical, electrical, or other hazardous equipment shall be made inaccessible to children. The Specialist observed exposed electrical wires on the playground. Corrective Action: Effective 5/22/2025, S1 stated she made a phone call to maintenance to fix electrical exposure issues, to ensure compliance with this regulation.

2.: Apparatus or Equipment: Based on observations on 5/22/2025 at 9:45 a.m., S1 failed to ensure a manufacturer's restraint device was available to be used by children in a one-year-old classroom. The Specialist observed a feeding table with one restraint device attached to the seating, other seats did not have any restraint devices attached. There were no footrest attached to the feeding table. Corrective Action: Effective 5/22/2025, S1 stated she pulled a smaller table into the classroom with smaller chairs for children to use during feeding time. S1 stated tables were ordered ,…

: Based on observations on 5/22/2025 at 9:30 a.m., S3 and S4 failed to ensure center's cribs were free of toys or other soft or loose bedding (including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges. The Specialist observed cribs with blankets and loose bedding. Corrective Action: Effective 5/22/2025, S1 stated she will order fitted sheets and remove all blankets from cribs, to ensure compliance with this regulation.

Infant Bibs: Based on observations on 5-22-2025 at 9:30 a.m., S3 and S4 allowed a bib to be worn by a child while asleep. The Specialist observed two infants sleep with bibs on. Corrective Action: Effective 5/22/2025, S1 stated she will make sure staff remove all bibs during nap time, to ensure compliance with this regulation.

Based on observation on 5/22/2025 at 9:45 a.m., S3 and S4 failed to ensure pacifiers were not attached to a child. The Specialist observed three infants with pacifiers attached to their clothing. Corrective Action: Effective 5/22/2025, S1 stated she would remove all pacifier holders when infants arrive to the center, to ensure compliance with this regulation.

1919.J. Based on observations/interviews on 5/22/2025 at 9:30 am, S3 and S4 failed to have bottle formula/breast milk for infants labeled with the child's name. The Specialist observed 4 out of 5 infants without labeled bottles. The Specialist informed S3 and S4 that bottles should be labeled for all children. Corrective Action: Effective 5/22/2025, S1 stated she will inform staff to cover names with clear tape to preserve ink applied when labeling, to ensure compliance with this regulation.

Based on record review and interviews at 12 p.m., S1 failed to notify the Department and Child Welfare within 24 hours or the next business day of the following critical incident : On 12/4/2024, O1 informed S1 that C1, four-years-old, stated S5 hit him. S5 denied the allegations. Corrective Action: Effective 4/24/2025, S1 stated going forward, she will report all allegations to the Department and Child Welfare, regardless if they are true or false, to ensure compliance with this regulation.

Electronic Devices Policy- Based on observations/interviews on 3-27-2025 at 10:00 a.m., S2 and S3 failed to follow the Electronic Devices Policy. The Specialist observed television programming displayed in an infant classroom.

Based on record review at 11:45 p.m., S1 failed to have documentation that 4 of 16 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S7 (DOH:9-9-2015) and S14 (DOH: 9-9-2016), S15 (DOH: 5-8-2020), S16 (DOH:10-1-2018) failed to have the current certification. S1 stated she will schedule a class for 4/15/2025.

1723. C.: Pediatric First Aid: Based on record review at 11:45 p.m., S1 failed to have documentation that 2 of 23 staff on the premises and accessible to children have current certification in Pediatric First Aid through training approved by the Department. S7( DOH:9-9-2015) and S14 (DOH: 9-9-2016), S15 (DOH: 5-8-2020), S16 (DOH:10-1-2018) failed to have the current certification. S1 stated she will schedule a class for 4/15/2025.

Based on record review at 11:00 a.m., S1 failed to have documentation that the entire center and play yard was checked after the last child departed to ensure that no child is left unattended at the center for 03-21-2025, 03-24-2025, 03-25-2025 and 03-26-2025.

: Based on observation/interview at 10:00am, S1 failed to provide written authorization from a physician for two infants to use a positioning device. The Specialist observed two infants sleeping in a swing.

1915.A. Health Services - Observation: Based on record review at 10:00 a.m., S1 failed to provide documentation of observations being documented when something is observed, noted on all children upon arrival to the center on 3-27-2025.

Based on record review on 2-13-2025 at 12:00 p.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed that the End-of Day-Check has not been completed since 2-5-2025.

Based on observation/ interview on 2-13-2025 at 12:00 p.m., S1 failed to ensure items that can be harmful to children, such as medications, poisons, cleaning supplies and chemicals, and equipment, tools, knives and other potentially dangerous utensils, were kept in a locked cabinet or other secure place that ensures they are inaccessible to children. The Specialist observed a gate on the toddler playground which is damaged, metal pole is cut in half with two sharp ends pulling away from the bottom of the gate.

Based on observation on 2-13-2025 at 11:40 a.m., S6 failed to ensure awake children did not remain in a crib, baby bed, swing, high chair, carrier or playpen for more than 30 consecutive minutes. The Specialists observed 2 one-year-old children standing in cribs during rest time.

1915.A. Health Services - Observation: Based on record review on 2-13-2025 at 11:35 a.m., S1 failed to provide documentation of observations being documented when something is observed, noted on children upon arrival to the center. Results including an explanation from parent and/or child were not documented. The Specialist observed that daily observations had not been completed by S6, S7, S8, S9 and S10 since 2-12-2025.
