Based on record review and interview at 4:15 p.m., S1 failed to provide documentation that the Department was notified within 24 hours of the following critical incident: On 12/18/2025, at 11:00 a.m., S1 was notified by S25 that C1, age two-years-old, lost his balance and fell on his arm. C1 was taken to the doctor and it was determined his arm was popped out of socket and fractured. Corrective Action: Effective 02/02/2025, S1 stated she will submit all critical incidents and verify with the Department that the CIR was received to ensure compliance to this regulation.

1807.B. Based on record review at 4:00 p.m., S1 failed to have documentation of eligible staff CCCBC determination available for S25 (DOH 12/18/2025) on the centerâs CCCBC roster at all times. This was not corrected prior the Specialist departure. Corrective Action: Effective 02/02/2026, S1 stated she will check the CCCBC roster monthly to ensure all staff have been added to the centerâs roster to ensure compliance with the regulation.

The Specialist observed 5 staff and 44 children (9 infants, ten 1-year-old, and twenty-five 2 to 3-year old children) present, two additional staff was needed to meet the ratios of the children present at the center.
