Based on record review at 11:30 a.m., S1 failed to provide opportunities for continuing education of staff members who are left alone with children, or who have supervisory or disciplinary authority over children. Eleven of eleven staff did not have 12 continuing education hours. This was not corrected prior to the Specialist departure. Corrective Action: Effective 5/5/2026, S1 stated she will schedule opportunities for continuing education for employees herself to ensure compliance with this regulation.

C.: Based on interviews at 2:00 p.m., S failed to notify the parent immediately of the following critical incident: On 4/14/2025, at around 10:25 a.m., C1, 2-years-old, was left unsupervised on the centers playground for about 30 seconds. An attempt to notify the parent was made at 11:18 a.m. This could not be corrected prior to the Specialist departure. Corrective Action: Effective 4/14/2025, S1 stated she was unaware that she needed to contact the parent immediately, but she will train S2 on the proper procedure to ensure compliance with this regulation.

Based on observations/interview at 10:25 a.m., S8 and S9 failed to ensure the children were supervised at all times while out on the centers playground. The Specialist observed, C1, 1-years-old, was left playing outside on the centers playground while S8 and S9 went back into the classroom with the remaining 16 children, ages 1 to 2-year-old. They failed to perform a headcount of the children in their care, which resulted in C1 being left on the playground alone for 30 seconds.
