Based on record review and interviews at 1:15 p.m., S1 failed to have documentation that 1 of 4 staff members completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. S1's training expired 1/31/2025. This deficiency is being re-cited. Corrective Action: Effective 6/25/2025, S1 stated she will complete this training by 6/27/2025, and going forward, she will have a reminder set 30 days prior to the expiration of the training to ensure compliance with this regulation.

A B.: Based on the record review/interviews on 05/21/2025 at 11:00 a.m., S1 failed to have documentation that 2 of 5 staff members completed the online Child Abuse and Neglect Mandated Reporter Training provided by DCFS, annually. S1 (DOH 12/22/2014), expired 1/31/2025 and S2 (DOH 01/22/2015) expired 1/12/2025. Corrective Action: Effective 05/30/2025, S1 stated that she will set a 30-day calendar reminder before the expiration of the training and make a notation in the staff's binders to ensure compliance with this regulation.
